Would writing "Accidentals missing in the source and added by the editor" only in square brackets be a bad approach? Should they be "ficta" as well? Quite hard to do in a piano part if it is a chord where one note is missing the sharp, for example. If the accidental is obvious, ...

What would be the reason for Bach to use a centered beam in bar 1 and not in bar 2? The music is the same.

If I understand you correctly about middle line stem directions, they can actually be set in the preferences to always point down.

I agree that the accent thing is bad. I solved that for one project by creating a custom playing technique with an accent always placed outside staff.
